Your first visit
Before you come to see us at Findon Road Dental Clinic, Woodville please read below to ensure you have everything you need.
Before you arrive
If you are attending for the first time, please try to arrive at your appointment at least 10 minutes before your scheduled appointment so we can collect the information we need to create your patient file.
The practice has a car park alongside the practice building. The dental practice is on the corner of Findon Road, Woodville and a side street called Ryan Avenue. The car park is accessible via Ryan Avenue.
What to bring
Please bring a list of any medications and the dosage that you are taking, as we will wish to record this. This is important as some medications may impact on your treatment. We will also want to know if medications have changed since your previous visit.
If you are coming to us from another practice we can arrange the release of your records and radiographs to us. You will need to come to Findon Road Dental Clinic to sign a release authority. This can be done at the first appointment or you may drop in prior to your appointment so we may have the records when you first attend.
If you are in a private health fund please bring your card to each appointment. We accept all private health fund cards.
If you are a Veteran’s Affairs gold card holder, please inform us and please bring your card to your first appointment.
For private health fund card holders, we will electronically submit a claim for you. If the claim is successful, you will then pay the gap amount. Should the claim be declined you will need to pay the full amount and subsequently submit a claim yourself.
We require full payment at each visit. Estimates can be issued following a full examination and treatment plan. We accept payments by: EFTPOS, Visa, MasterCard, Amex, Cheque or Cash.
Child Dental Benefits Scheme and SA Dental Service referrals
If you are making an appointment for your child and your child is eligible for the Medicare sponsored Child Dental Benefit Scheme, please inform us of that fact and provide us with his/her Medicare number as we are required to confirm eligibility as well as available allowance. Please note that currently we are bulk billing treatment under the Child Dental Benefits Scheme so no gap is required for Medicare schedule treatments while the child has available allowance.
We also accept patients who present written authority from the SA Dental Service for treatment under General Dental, Emergency and Denture Schemes. Please inform us of this when making your appointment.
Appointment changes and enquiries
Should you need to change your appointment time please give us at least one business day’s notice. This will give us the opportunity to allocate the time to someone else. There may be a fee applied to a missed appointment..
